Summary:
The Ambulatory Care Pharmacy Professional is someone who prides themselves on their attention to detail and customer service.
An in-depth understanding of the pharmacy technician position and familiarity with the concept of formulary lists, utilization management programs and pharmacy renewal protocols is necessary to make informed decisions regarding medication management.
This position requires that the applicant be agile and have the ability to change course easily and without getting frustrated.
The applicant should be able to independently manage their time in a fast-paced environment.
Qualifications:
Certified Pharmacy technician with at least seven years of retail pharmacy experience or at least 3 years working in a value-based care or health system Ability to integrate with the healthcare team and work collaboratively to establish and maintain effective working relationships with other healthcare professionals, practitioners, patients, and families Working knowledge of CMS HEDIS measures, especially as they relate to Part D Proficient at working with or learning how to work with Electronic Health Record (HER) systems Working knowledge of the use of enterprise data warehouses and patient registries Understanding of the principles of clinical integration and population health management Effective oral, written communication, and telephone skills.
Good organizational skills to handle multiple priorities while remaining professional and calm.
Excellent time management skills and ability to meet tight deadlines Proficient in Microsoft Office Suite with demonstrated expertise in Microsoft Excel Ability to work with many diverse people, including elderly, disabled, mentally challenged, low literacy, impoverished, and underserved populations.
Ability to work independently and perform functions with minimal supervision.
Ability to multi-task and prioritize responsibilities and objectives.
GENERAL DUTIES AND
Responsibilities:
Review pharmacy medication refill requests and utilize protocols and algorithms to either refill a medication for appropriate timespan or refer to the provider or clinical pharmacist for appropriate follow-up.
Conduct outbound medication adherence member calls to address gaps in care and improve health care outcomes under Clinical Pharmacist's supervision Review, collate and maintain HEDIS adherence gap reports and ensure follow-up on all patients for assigned clinics Communicate with provider offices regarding medication issues and patient concerns in a manner approved by clinic locations and Clinical Pharmacist approval Effectively communicate and collaborate in a clear and concise manner to cl Support Clinical Pharmacist with ad-hoc projects and perform other duties as assigned.
Support Clinical Pharmacist documentation and follow-up as required Ensure compliance with state and federal laws and applicable rules and regulations for clinical pharmacy practice.
Maintain professional and technical knowledge, licensures and certifications through continuing education Maintains and promotes HIPAA compliance.

High School Diploma Completion of a Certified Pharmacy Technician program MINIMUM EXPERIENCE REQUIRED:

Minimum of 7 years working experience in a retail pharmacy setting MINIMUM LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED BY REGULATION OR POLICY:
Licensed as a pharmacy technician in the state of Florida required National Pharmacy Technician Certification required Immunization Certification preferred Quick Apply.
