Summary:

Under the direction of the Site Supervisor, the Quality Improvement Coordinator for the Hoag Medical Group - HMTS performs a variety of duties in support of the Quality Improvement activities, ensuring even patient flow and continuous communication within HMG-HMTS, referring physician offices, and Organizational staff.
The Quality Improvement Coordinator displays and practices a work-style that reflects the mission, vision and values of the Hoag Medical Group as well as Hoag Hospital.
Participates in the continuous Quality Improvement process, maintains a working knowledge of departmental standard operating procedures, including the use of specialized instrumentation, Quality Control requirements, and preventive maintenance.

Essential Functions

  • Pull the daily quality schedule report to distribute to all Medical Assistants (MA)/providers to facilitate a daily morning review of all scheduled patients with appointments due for screenings and tests to close quality care gaps.
  • Will ensure that every MA/provider team has reviewed the schedule reports of who is scheduled to come in with care gaps to complete some of the necessary tests or screenings.
  • Throughout the day, reviews quality reporting system to search for patients who have created sameday appointments and are due for screenings and tests to close quality care gaps.
  • Under the guidance of the Quality Improvement Specialist, leads and coordinates special projects aimed at closing quality care gap reports specific assigned site(s).
    Special projects will include a more frequent review of specific measures within monthly patient care gap report in order to review more charts and contact more patients who are in need of screenings.
  • Support provider and medical assistant teams with daily Quality Improvement tasks such as comparing schedule with due list(s), downloading point of care report(s), and preparing per protocol orders.
  • Analyzes the monthly patient care gap report per provider at each designated site(s) in order to:
  • Contact patients on list by phone, letter, or electronically to remind patients of their health screenings or tests due.
    Will make at least three (3) attempts to contact patient in the attempts of encouraging completing necessary tests and screenings to close care gaps.
    Interaction with patients is documented in Electronic Medical Record (EMR) system.
  • Review patient chart in EMR for any tests or screenings completed within the quality measure guidelines.
    Will search in other systems within EMR and hospital EMR to locate any record(s) that demonstrate completion of screening or testing.
  • Review patient s previous history and referrals that may indicate that tests or screenings have been completed by provider outside of network.
    Will work on contacting provider(s), obtain a medical release from patient, to retrieve the necessary records form provider(s) outside of network.
  • Daily report will be sent to Quality Improvement Specialist with progress on patient care gap report that will show daily productivity.

Education, Training and Experience

Required:
Medical Assistant

  • High school graduate or equivalent
  • Graduate of Medical Assistant program
  • Minimum one (1) year previous medical back-office experience, preferably within an acute healthcare system environment

Preferred:

  • Associate or Bachelor s degree in healthcare-related field

Skills or Other Qualifications

Required:

  • Excellent communication skills; customer focused; excellent attention to detail; able to prioritize tasks and meet deadlines; strong interpersonal skills; able to work in a team environment.
  • Basic keyboarding skills at 35 WPM.
  • Basic knowledge of Microsoft Office Suite, including Outlook, Word and Excel.
  • Proven ability to interface with all customer levels (physicians, Organizational staff, insurance companies, patients and family members).
  • Basic knowledge of medical terminology

Preferred:

  • Qualified bilingual or multilingual candidates

License and Certifications

Required:

  • Certification as a Medical Assistant by an accredited program
  • BLS certification

Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
